Localities

At the type locality, the Szklarska Poręba Huta granite quarry, Szklarska Poręba, Karkonosze County, Lower Silesian Voivodeship, Poland, kozłowskiite was found as a band in the core of a zoned silesiaitekristiansenite crystal from the granitic pegmatite (MM 86.3.507-517).
