At the type locality, Valachov Hill, Skřivaň, Pavlíkov, Rakovník District, Central Bohemian Region, Czech Republic, slavikite is an uncommon secondary oxidation product of pyrite in shales and slate. Associated minerals include halotrichite, pickeringite, fibroferrite, copiapite, melanterite, epsomite, jarosite and gypsum (HOM).
