At the type locality, the Koashva Open Pit, Koashva Mt, Khibiny Massif, Murmansk Oblast, Russia, koashvite occurs as veinlets replacing lomonosovite in an ultra-agpaitic pegmatite in an alkalic massif.
Associated minerals include lomonosovite, pectolite, villiaumite and natrophosphate (HOM).
