Itoite is a very rare mineral that occurs at the type locality, the Tsumeb Mine, Oshikoto Region, Namibia, in the upper oxidation zone of a germanium-bearing copper and arsenic rich polymetallic ore deposit, as fine-grained pseudomorphs after fleischerite, associated with cerussite, mimetite and altered tennantite, and also as a crust on plumbojarosite and mimetite on dolomite (AM 45.1313, Mindat, HOM).
