At the type locality, the Clara mine, Oberwolfach, Ortenaukreis, Freiburg Region, Baden-Württemberg, Germany, cualstibite occurs as a secondary mineral from oxidation of the hydrothermal polymetallic barytefluorite deposit. Associated minerals include cornwallite, arsenogoyazite, goethite, baryte, fluorite and quartz (HOM).
