The type locality, the Tsumeb mine, Tsumeb, Oshikoto Region, Namibia, is the only known locality for fahleite to date (March 2021). Here fahleite was found on oxidised tennantite – chalcocite ore from a deep oxidised zone of a dolostone-hosted hydrothermal polymetallic ore deposit (HOM). Associated minerals include lavendulan, cuprian adamite, conichalcite, tsumcorite, tennantite, chalcocite, quartz, calcite, gypsum and smolyaninovite ( HOM, Dana).
