At the type locality, the Moldava deposit, Moldava, Teplice District, Ústí nad Labem Region, Czech Republic,
tetrarooseveltite is a rare secondary mineral in vugs in
an oxidised
fluorite–baryte–quartz
vein. Associated minerals include bayldonite,
malachite, mimetite,
fluorite, quartz,
preisingerite, zavaritskite,
bismutite, rooseveltite,
mixite and wulfenite
(HOM, Dana).
