Tetrarooseveltite is a very rare mineral that to date (December 2025) has been found only at the type locality.

Localities

At the type locality, the Moldava deposit, Moldava, Teplice District, Ústí nad Labem Region, Czech Republic, tetrarooseveltite is a rare secondary mineral in vugs in an oxidised fluoritebarytequartz vein. Associated minerals include bayldonite, malachite, mimetite, fluorite, quartz, preisingerite, zavaritskite, bismutite, rooseveltite, mixite and wulfenite (HOM, Dana).
