Localities

At the type locality, the Jakobsberg mine, Jakobsberg ore field, Nordmark District, Filipstad, Värmland County, Sweden, hematophanite occurs in a metamorphosed banded iron-manganese ore deposit in dolostone (Webmin). Associated minerals include plumboferrite, jacobsite, andradite, copper, cuprite, cerussite and calcite (HOM, Dana).

At the Kombat Mine, Kombat, Grootfontein, Otjozondjupa Region, Namibia, hematophanite is associated with copper, jacobsite, hematite and damaraite (HOM).
